Ayers Insurance Agency of AZ, LLC is not connected or affiliated with any Government Agency. Enrollment in these plans is dependent on contract renewal. Plans are insured or covered by a individual Insurance Companies, Medicare Advantage organization(s) with a Medicare contract and/or a Medicare-approved Part D sponsor. Enrollment in the plan depends on the plan’s contract renewal with Medicare. Every year, Medicare evaluates plans based on a 5-Star rating system. You are not required to provide any health related information unless it is used to determine enrollment eligibility.
